Terms and Conditions

• I hereby represent that I am physically fit to receive and participate in the prescribed course of instruction.
• I fully understand and acknowledge that: There are risks and dangers associated with participation in martial arts events and activities which could result in bodily injury partial and/or total disability, paralysis and death. The social, financial and economic losses and/or damages, which could result from these risks and dangers described above, could be severe. I understand that the practice of Martial Arts involves the risk of serious injury and financial/economic loss and I participate on the understanding that I am doing so at my own risk.
• I agree to and hereby indemnify White Lotus Muay Thai and its officers, instructors, servants and each of them from and against claims of damage, personal injury or illness sustained by myself or my child while participating in any fitness activities or functions connected with White Lotus Muay Thai or travelling to and from such activities or functions arising from any breach of statutory duty.
• The applicant for him/herself, his/her executors, administrators, dependants, and other personal representatives, hereby absolves and indemnifies the providers and all their servants, agents, employees and other students or persons under the providers control (the "indemnified") from all liability howsoever arising for injury or damage (including but not limited to the applicants person, whether fatal or otherwise, property and personal belonging) however caused including by the negligence of the indemnified, arising out of or participating in Martial Arts or in any way caused by, or arising out of, any activities carried on by the indemnified.
• Photo, Video Audio Release: I agree that White Lotus Thai reserves the right to take video and audio material of student classes and school events, and that these materials may be used for display, promotion and or advertising, or sold for profit, and I hereby waive my rights to any compensation arising out of my appearance in such materials.
• I understand that failure to attend training does not relieve me of my obligation to pay in full.

I declare that all of the information I have provided is true and correct.

1. I agree to abide by the rules and regulations set forth by White Lotus Muay Thai.

2. I acknowledge that my membership may be terminated at any time by White Lotus Muay Thai if I violate any of its rules and regulations or fail to follow the instructions of my instructors.

3. In the event of an accident or injury, I authorise White Lotus Muay Thai instructors to perform necessary first aid and/or call for emergency services as needed.

4. I take full responsibility for any medical or emergency services incurred on my behalf as a result of an accident or injury that occurs during my participation in activities held by White Lotus Muay Thai.

5. I understand and acknowledge that White Lotus Muay Thai recommends seeking medical advice before beginning any training or exercise program, and that I am fit to participate in the activities offered by White Lotus Muay Thai.

6. White Lotus Muay Thai reserves the right to refuse or cancel my membership at any time, without providing a reason.
